New Client Form

Creekside Veterinary Hospital may use an audio recording device during your appointment(s) to assist our veterinary team in generating accurate clinical notes and medical records for your pet. Recordings are used solely for this purpose, are handled in accordance with our clinic's privacy practices, and are not shared outside our clinic except as required to provide your pet's care or as required by law. You may decline this consent, or withdraw it at any time by notifying clinic staff, without affecting the veterinary care your pet receives.

Payment is due in full at the time services are performed. If your pet is admitted to the hospital, care begins once you've signed the consent and estimate form and provided an initial deposit of 50% of the upper end of the estimate. We accept major credit cards, debit, cash, and e-transfer, and offer financing through Petcard via iFinance Canada. We don't extend in-house credit or billing, and outstanding balances may be sent to collections if no prior arrangement has been made.
