Surgery, Dental & Anesthesia Consent Form

Your pet's dental procedure will be performed under general anesthesia. As with any anesthetic procedure, there's a small risk of complications such as an adverse reaction, changes in blood pressure or heart rate, breathing difficulty, or — rarely — aspiration pneumonia. If extractions are needed, additional risks can include bleeding, jaw fracture in small or fragile-boned pets, retained root fragments, or a rare opening between the mouth and nasal cavity. Afterward, some pets experience mild pain, swelling, infection at the site, or a temporary change in how they chew.

Dental cleaning under anesthesia is included in your estimate; extractions add cost (enquire for cost). Because dental disease often extends below the gumline, your final treatment plan and cost may change based on what we find during the exam and X-rays.

Spaying removes a female dog or cat's ovaries and uterus to prevent pregnancy, lower certain cancer risks, and stop heat cycles. It's routine and generally safe, but like any surgery carries some risk: a small chance of an anesthesia reaction, infection at the incision, minor (rarely significant) bleeding, expected post-surgical discomfort managed with pain medication, and occasional healing issues at the incision site. Some pets have a mild medication reaction, and in rare cases urinary incontinence can develop afterward due to hormonal changes. Very rarely, complications may affect nearby organs or require further treatment.

Neutering removes a male pet's testicles to prevent reproduction, reduce certain behaviors, and lower the risk of conditions like testicular cancer. It's common and generally safe, with standard surgical risks: a small chance of an anesthesia reaction, minor bleeding, infection at the site, temporary discomfort or swelling managed with pain medication, and a typical recovery that may include grogginess or mild digestive upset. Rarely, healing issues, retained tissue requiring further surgery, or urinary tract issues can occur.

This procedure removes a skin or under-the-skin mass under general anesthesia. Risks include the standard risks of anesthesia, bleeding, infection at the surgical site, the incision reopening (especially in high-motion areas), the mass regrowing if removal is incomplete, and, depending on location, a small chance of nerve involvement. A biopsy is recommended in most cases to confirm the diagnosis.

This procedure surgically corrects a prolapsed third eyelid gland under general anesthesia. Risks include the standard risks of anesthesia, the gland prolapsing again, infection or inflammation at the site, a small chance of dry eye requiring lifelong medication, a reaction to suture material, or the gland shrinking over time.
